Dr J. Palmhert - Paediatric Surgeon - Red Cross Children's Hospital - Rondebosch, Cape Town, South Africa "To: The Chairman - Scientific Programme Committee - ISBI Congress 1998 - Jerusalem
- An abstract of a paper entitled "The use of
BURNSHIELD® management"
Early burn wound management influences the healing processes and eventual outcome of the burn wound. Following on a previous animal study in this institution, comparing
BURNSHIELD® (melaleuca alternifolia gel) to other forms of treatment in the acute management of burn wounds, a study was conducted to evaluate the efficacy of BURNSHIELD® dressing in paediatric patients.

Eleven patients (7 male, 4 female; mean age 34 months, range 7-149) with thermal injuries (10 hot water burns (HWB), 1 fire; with a mean TBSA burn of 11.3%, range 7.5-18) were admitted within 10 hours of burn and were treated with
BURNSHIELD® dressing for the first 10 hours of admission. Thereafter BURNSHIELD® was removed and routine management of the burn wounds instituted.

Twelve patients (5 male, 7 female; mean age 31 months, range 7-116) with thermal injuries (8 HWB, 4 fire; with a mean TBSA burn of 15.2%, range 5-35) served as a control group. They received Betadine or Flamazine dressings on admission followed by routine burn wound management.

The patients treated with
BURNSHIELD® dressings, the mean hospital stay was 9.9 days (range 3-33) and mean wound healing time 24.7 days (range 12-63), with only 4 patients requiring skin grafting. The mean hospital stay for the control group was 17.2 days (range 4-33), and mean wound healing time 28.2 days (range 12-62) with 6 patients requiring skin grafting.

Preliminary data would indicate that the cooling down effect of
BURNSHIELD® has a beneficial effect on the outcome of burn wounds.

"The anti-bacterial effect of the Melaleuca alt. has prompted an investigation into the re-formulation of the gel into an ointment or cream for use as a topical anti-microbial agent. Initial investigations are already proving promising." (extract)
Prof. Rode, University of Cape Town - Department of Paediatric Surgery - Institute of Child Health An evaluation of the first aid cooling and healing effect of Melaleuca gel "BURNSHIELD®"; A new burn dressing BURNSHIELD® is an effective alternative in the first aid cooling of burns and has a promotive effect on healing. Its application still has benefit if applied 30 minutes after the burn has occurred. It is a light, easily transportable sterile dressing. A single application of this dressing is an effective first aid measure in the acute burn. The mechanism of action is probably the combination of cooling, healing promoting properties, and an anti-bacterial effect" (extract)
